Elation Patient Passport
The Elation Passport system allows your provider to share information with you in a secure way. Whether information from your chart, results of a test, or simply answering your questions. You are free to contact your provider in whatever way you wish including text, email, and phone; however, when your provider needs to respond with information protected under HIPAA, it will be by phone or through the Passport. To access the passport, click on the button below. If you have any questions about how to use it, don’t hesitate to contact us.
Hale Health App
The Hale Health app of iOS and Android is another way to communicate with us - you do not need to use it unless you want to. The app will allow you to text message us securely (which allows us to respond to you by text). It can also ask you some questions about your medical condition to help you organize your information saving time for you and for us. You are also able to send us pictures through the app.
